We reviewed 43 eyes of idiopathic macular hole which underwent vitreous surgery. We paid particular attention to the postoperative visual field defect. The intraoperative perfusion pressure was kept below 50cm of water column and the fluid/air exchange was performed at 30 mmHg. Postoperative visual field developed or progressed in 2 eyes (4.7%) , including one eye with glaucoma. These findings show the efficacy of our intraoperative procedure in preventing postopera-tive visual field defect. Eyes with fragile optic nerve head, as in glaucoma, need due attention in this aspect.
