糖尿病患者の両眼性乳頭腫脹

Bilateral disc swelling in a diabetic patient

　62歳男性の糖尿病患者に，視力良好な両眼性乳頭腫脹を認めた。右眼は著明な発赤腫脹，視野の鼻下側欠損を呈したが，左眼は限局的で軽微な浮腫で正常視野を示しdiabetic papillopathyが疑われた。経過により右乳頭腫脹は徐々に消退したが，左眼は増悪し，ピークとなった2か月後に視力低下（0.02）と中心暗点をきたした。ステロイド治療により左眼矯正視力は0.5まで回復し，視野は鼻側から鼻下側にかけて沈下を残した。結局，本症例は乳頭腫脹が視力障害に先行した両眼性の前部虚血性視神経症（AION）と診断された。糖尿病患者の乳頭腫脹は微小循環不全によって生じた軸索流のうっ滞によるものであり，軽度の虚血の持続により不可逆的軸索障害をきたすことが示唆される。したがって，diabetic papillopathyと虚血性視神経症は同一の病態である可能性がある。

A 62-year-old male presented with bilateral edema of the optic disc. He had been under treat-ment for diabetes during the foregoing 12 years. He manifested simple retinopathy with normal visual acuity. The optic disc in the right eye appeared markedly hyperemic and swollen. Visual field defect was present inferonasally. The optic disc in the left eye showed minimal localized edema. The visual field was intact. These clinical pictures werecompatible with diabetic papillopathy.

Two months later, the disc edema in the left eye progressed with visual loss and central scotoma. Systemic corticosteroid therapy was followed by improvement in fundus findings and visual acuity. Retrospectively, this case was diagnosed as bilat-eral anterior ischemic optic neuropathy (AION) preceded by asymptomatic disc edema. Diabetic papillopathy seemed to share a common path-ogenesis with AION.
