A 37-year-old male was referred to us for diabetes detected 11 years before. His visual acuity was 1.0 in either eye. Both eyes showed preproliferative diabetic retinopathy. His right eye developed branch retinal artery occlusion (BRAO) 52 days and 57 days after panretinal photocoagulation. His left eye developed BRAO 15 days after additional photocoagulation. Laboratory studies showed elevated level of anticardiolipin antibody and led to the diagnosis of antiphospolipid antibody syndrome. It appeared that BRAO induced by antiphospholipid antibody syndrome as the underlying lesion and triggered by panretinal photocoagulation and drastic decrease in blood sugar level.
