肥厚性硬膜炎による両眼視神経症の1例

Bilateral optic neuropathy in a case of hypertrophic pachymeningitis

要約　目的：両眼に視神経症が発症した肥厚性硬膜炎の症例の報告。症例：65歳女性が3週前からの視力低下と視野狭窄で受診した。所見：矯正視力は右0.2，左手動弁であった。ガドリニウム造影磁気共鳴画像（MRI）検査で前頭部を中心に脳硬膜の肥厚があり，視神経周囲に高信号があった。硬膜の生検で炎症細胞の浸潤があり，肥厚性硬膜炎と診断した。ステロイドパルス療法とこれに続くプレドニゾロン内服で，硬膜の肥厚は減少し，視力は4か月後に1.0に回復し，視野狭窄も軽快した。結論：高齢者の球後視神経炎が肥厚性硬膜炎によることがある。診断には造影MRI検査が有用であった。

Abstract.　Purpose：To report a case of hypertrophic pachymeningitis with bilateral optic neuropathy as the initial manifestation. Case：A 65-year-old female presented with impaired visual acuity and field defect since 3 weeks before. Findings：Corrected visual acuity was 0.2 right and hand motion left. Gadolinium-enhanced MRI showed thickening of dura mater in the frontal lobe and intense signal around the optic tract. Biopsy of the dura led to the diagnosis of hypertrophic pachymeningitis. Pulsed corticosteroid therapy with tapering prednisolone was followed by decreased thickness of dura mater, improved visual acuity to 1.0 in either eye, and normalization of visual field. Conclusion：This case illustrates that hypertrophic pachymeningitis may show symptoms simulating retrobulbar neuritis. Enhanced MRI may be useful in the diagnosis.
