未熟児網膜症における血圧の関与

Implication of systemic blood pressure in retinopathy of prematurity

（19-C-1）　1996年までの6年間に，名古屋市立城北病院新生児集中治療室で管理された，出生体重1,500g未満の160例を対象とし，前半の60例と後半の100例に分けて比較検討した。後半で未熟児網膜症（ROP）の発症，重症化率は少なく，生後早期に低血圧を示した症例の割合も減少していた。低血圧がROPに与える影響を検討したところ，全症例において低血圧例にROPの発症，重症化が有意に多くみられた。以上から，近年の未熟児の循環管理の向上が，ROPの発生，進行を抑制しており，また低血圧はROPの危険因子であることが示された。

We reviewed a series of 160 babies with the birthweight of 1,500g or less treated in Nagoya Municipal Johoku Hospital during 6 years (1991-96). The incidence of manifestation and severity of retinopathy of prematurity (ROP) was less in the 100 cases during the latter 3 years. The incidence of systemic hypotension for the early period after birth was also less during the same term. In the total 160 cases, the incidence of ROP and severe ROP was more frequent in babies with systemic hypotension. An improved neonatal care seemed to be the underlying cause. Systemic hypotension during the neonatal period is one of the risk factors for ROP.
