A 73-year-old male underwent cataract surgery using phacoemulsification and in-the-bag fixation of silicone intraocular lens (IOL) in his right eye. Retinal detachment surgery had been performed in the same eye 9 years before. The anterior capsular started to contract 2 months later and was successfully treated by anterior capsulotomy with Nd: YAG laser. Posterior capsulotomy with Nd:YAG laser was performed another 2 years later. Subluxation of IOL developed 3 years after IOL implantation due to further contracture of the lens capsular and zonular dialysis. Full visuals acuity was obtained after extraction of the lens capsule with IOL, anterior vitrectomy and sulcus fixation of another PMMA IOL.
