シリコーン眼内レンズ挿入眼での液空気置換時の眼底視認性

Visibility of the fundus during fluid-air exchange through silicone intraocular lens

　シリコーン眼内レンズ挿入眼の網膜剥離に対し，後発白内障切開術を行った後に，硝子体手術を行い復位を試みた。術中，液空気置換を行ったところIOL（intraocular lens）裏面に水滴が多数付着し，眼底観察が困難になった。後発白内障切開術後のシリコーンIOL挿入眼における硝子体手術では空気灌流下において眼底の視認性が低下するので，白内障手術時のシリコーンlOL挿入はその適応を十分に考慮すべきと考えられた。また必要に応じてあらかじめIOLの摘出も考慮に入れた上で手術を行うべきと考えられた。

We performed vitreous surgery for rhegmatogenous retinal detachment in an eye which had undergone silicone intraocular lens (IOL) implantation and laser posterior capsulotomy. While the vitreous cavity was filled by air, fiuid droplets condensed on the posterior surface of silicone IOL and interfered with the visibility of the fundus. Implantation of silicone IOL will have to be decided with due precaution in eyes which may later neces-sitate vitreous surgery.
