落屑症候群を伴う白内障手術患者の長期予後

Longterm course after cataract surgery in eyes with pseudoexfoliation syndrome

　落屑症候群に伴った白内障術後患者の長期予後について検討した。対象は落屑症候群と診断され，術後12〜40か月を経過した白内障術後患者15例20眼で，対照は白内障以外に眼科的疾患を持たない，同時期に白内障の手術を受けた34例53眼である。20眼中1眼は，術前より眼圧が高く，薬物療法および隅角線維柱帯形成術によっても眼圧が低下せず，線維柱帯切除術を同時に施行した症例であった。最終観察時点の視力は60％が0.8以上であったが，2眼（5％）が0.3であった。その1眼は線維柱帯切除術を同時に施行した症例であった。眼圧は落屑症候群，対照群ともに術後有意（p＜0.05，p＜0.001）に術前より低下していた。

We reviewed 20 eyes of 15 patients who underwent cataract surgery. All the eyes had pseudoexfoliation syndrome. One eye had glaucoma necessitating simultaneous trabeculectomy. The final visual acuity 12 to 40 months after surgery was 0.8 or better in 60% of the eyes. It was 0.3 in 2 eyes (5%) , including the case with glaucoma. The intraocular pressure (I0P) was significantly lower than the preoperative value (p ＜ 0.05) . In the control series of 53 eyes which did not have pseudoexfoliation and which underwent cataract surgery during the same period, the IOP was also significantly lower than the preoperative value (p ＜ 0.001) .
