新鮮角膜を用いた治療的角膜移植の成績

Results of therapeutic keratoplasty using fresh donor cornea

　新鮮角膜を用いて治療的角膜移植を行い，3か月以上経過観察できた33眼についてその転帰を検討した。疾患の内訳は細菌性角膜潰瘍8眼，真菌性角膜潰瘍4眼，角膜ヘルペス6眼，周辺部角膜潰瘍5眼であった。全症例で治療的角膜移植の目的を達成でき，透明治癒率67％．視力改善率61％の成績が得られた。新鮮角膜を用いた治療的角膜移植術は，角膜の補強と病巣の除去とともに視力予後でも有用であった。

We reviewed 33 cases of therapeutic keratoplasty using fresh donor cornea followed up for 3 months or longer. The causative lesions were bacterial ulcer 8 eyes, fungal corneal ulcer 4 eyes, herpetic corneal ulcer 6 eyes and peripheral ulcerative keratitis 5 eyes. These eyes were treated by either penetrating or lamellar keratoplasty. Transparent graft was attained in 67%. The visual acuity improved in 61%. The findings show the value of therapeutic keratoplasty using fresh donor cornea for active keratitis or corneal perforatoins that fail to respond to medical therapy.
