再発性翼状片に対する羊膜および自己輪部移植術

Transplantation of amniotic membrane and limbal autograft in the treatment of recurrent pterygium

　数回再発を繰り返している再発性翼状片の63歳と67歳の2例2眼に手術を行った。両症例とも瞼球癒着と外転障害があった。翼状片と癒着組織を切除した後に，強膜露出部に冷凍保存羊膜を縫着し，さらに患者眼の上方結膜を輪部を含んで採取し，羊膜上に縫着した。羊膜を使った理由は，抗炎症効果と血管新生抑制効果があり，抗原となりにくいことにある。両症例とも術後の再発はなく，眼球運動は正常化した。今回の手術術式は，難治性翼状片の治療に有効であった。

We surgically treated recurrent pterygium in two eyes of two patients aged 63 and 67 years. Both eyes showed symblepharon and restricted abduction. After excising the pterygium and separating the symblepharon, the bared sclera was covered by preserved amniotic membrane. This area was further covered by a piece of bulbar conjunctiva from the same eye. The amnion was used, as the rationale, because of its antiinflammatory effect, inhibition of neovascularization and absence of antigenicity. There was no recurrence after surgery. The ocular motility became normalized. The present surgical approach was thus useful in treating recurrent pterygium.
