Two cases with ocular pain as the chief complaint were diagnosed as unilateral iris tumor. Further studies led to the detection of lung carcinoma as the underlying cause. Both were males aged 82 and 52 years each. One had small cell lung carcinoma and the other had adenocarcinoma of the lung. In the second case, atypical cells consistent with adenocarcinoma were detected in the aqueous. The carcinoembryonic antigen (CEA) and carbohydrate antigen (CA15-3) values were elevated in the aqueous and were normal in the serum. This case illustrates the usefulness of cytological studies and tumor markers in the aqueous in the diagnosis of metastatic iris tumor.
