We quantitated the lens opacity after vitrectomy using an anterior eye segment analysis system in consecutive 103 eyes. Lens opacity 6 months after vitrectomy was more positively correlated with the preoperative opacity than with the age. Postoperative progression of lens opacity was related with the sex (i.e. female) and with the use of gas tamponade and not with the duration of surgery. Eyes with proliferative diabetic retinopathy showed less progression in lens opacity than in other diseases including idiopathic macular hole. Above findings serve as guidelines in deciding simultaneous lens removal during vitrectomy.
