12歳以下の小児のヘルペス性角膜炎16眼の病型や治療経過につき病歴から検討した。初発病型は上皮型7眼(44%),実質型6眼(37%),結膜炎型3眼(19%)であり,従来の報告よりも実質型が多かった。全症例における20回の再発のうち,上皮型が7回(35%),実質型が13回(65%)であり,実質型での再発が多かった。最終的に12眼(75%)で0.5以上の視力を保持できたが,強度の角膜混濁のため0.1未満の視力低下が2眼(13%)にあった。小児期のヘルペス性角膜炎ではアシクロビルを中心とした抗ウイルス薬を適切に使用し,視力障害を避けることが重要である。
We reviewed the clinical course of herpetic keratitis in 16 children under the age of 12 years seen during the foregoing 18.5 years. All the cases were unilaterally affected. The primary lesions manifested as epithelial keratitis in 7 eyes (44%), stromal keratitis in 6 (37%) and conjunctivitis in 3 (19%). Out of the total of 20 recurrences, epithelial lesions recurred 7 times (35%) and stromal lesion 13 times (65%) . As the final outcome, corrected visual acuity of 20/40 or over was obtained in 75% of eyes and visual acuity of less than 20/200 in 13% of eyes due to corneal scarring. A proper use of antiviral agents, including acyclovir, was important for good visual outcome in the management of herpetic ocular lesions in children.
