We perfomed cataract surgery on both eyes of a 70-year-old male who had received radial keratotomy four years before. The power of intraocular lenses (IOLs) were determined by standard keratometric values. Immediately after surgery, both eyes showed hyperopia by +6.0 and +4.5 diopters in each eye, necessitating exchange of IOL two weeks later. It appeared that the keratotomic scars induced errors in predicting the power of IOL by keratometry. Transient corneal flattening due to postoperative edema appeared to be another factor. We advocate videokeratography in calculating the IOL power in eyes after radial keratotomy.
