ルアー針による角膜穿孔ならびに網膜下血腫の1例

A case of corneal perforation and subretinal hematoma caused by lure fishing hook

（展示131）　20歳男性が魚釣中に，ルアー針が左眼に刺入した。2時間後に受診し，角膜穿孔，ルアー針の先端の前房内留置，前房出血があった。視力は20cm指数弁であった。異物の除去と角膜縫合を行った。翌日に網膜下血腫が発見された。受傷後10日目にtissue-plasminogenactivatorを併用する網膜下血腫除去術を行い，視力は0.2に回復した。脈絡膜破裂による外傷性網膜下血腫にこの手技が有効であった1例である。

A 20-year-old male was injured by a fishing hook in his left eye during lure fishing. When seen 2 hours after the injury, the left eye showed corneal perforation, hyphema and retention of the tip of the lure hook in the anterior chamber. The visual acuity was 20 cm counting/fingers. The eye was treated by removal of the hook with corneal suture. Subretinal hematoma was detected the following day. We performed vitrectomy and removed the hematoma using tissue-plasminogen activator. The visual acuity improved to 0.2. The case illustrates the value of this surgical approach for posttraumatic subretinal hematoma caused by choroidal rupture.
