We performed vitreous surgery in 36 eyes of idiopathic macular hole. The series comprised one eye of stage 1, 8 eyes of stage 2,21 eyes of stage 3 and 6 eyes of stage 4 holes. Closure of macular hole was obtained in 31 eyes (86%) . The hole did not close in 2 eyes of stage 2, 2 eyes of stage 3 and 1 eye of stage 4 hole. Visual acuity improved in 33% of eyes of stage 4 and in 90% of eyes of stage 1, 2 or 3 holes. Visual acuity failed to improve in 2 eyes of stage 4 hole treated by additional autoserum. The visual outcome was not correlated with the duration of the disease. It appeared that the actual onset of macular hole did not necessarily coincide with the subjective perception. Measurement of foveal sensitivity by automated perimeter was handicapped by unstable fixation but was useful in evaluating the postsurgical recovery of visual functions.
