網膜動脈・静脈閉塞症における血漿エンドセリン活性値とCa2＋拮抗薬の効果

Plasma endothelin levels and effect of systemic calcium antagonist in retinal artery and vein occlusions

　エンドセリン（endothelin：ET）は，強力な血管収縮作用を持つ生理活性ペプチドである。眼循環障害による疾患として代表的な網膜動脈・静脈閉塞症とETとの関係を調べた。未治療の同疾患46例の血漿ET−1活性値を測定した結果，陳旧性網膜静脈分枝閉塞症・硝子体出血を伴った網膜静脈分枝閉塞症は，正常者に比べ特に高値を示した。ET−1眼循環障害を抑制し得るCa^2＋拮抗薬の全身投与による，網膜静脈分枝閉塞症への影響について検討した結果，対照群に比べて症状改善までの期間が短い傾向を認めた。これらの疾患の発症にET−1が関与している可能性が示され，またCa^2＋拮抗薬の内服は末梢循環改善薬としての臨床的有用性が示唆された。

We quantitated the plasma levels of endothelin-1, a potent vasoconstrictor peptide, in 46 patients with untreated retinal artery or vein occlusions. We observed significantly elevated levels in old branch retinal occlusion with or without vitreous hemorrhage than in normal controls. Fundus findings showed a more rapid improvement in patients who received systemic calcium antagonist which inhibits the ocular circulation disturbance by endothelin-1.
