角膜穿孔および角膜欠損の角膜移植

Penetrating keratoplasty in acute corneal perforation and traumatic laceration corneal defect

　穿孔性角膜炎に対し，強膜半層円板と生体接着剤（アロンアルファA）にて穿孔を塞ぎ，前房形成を待ってトレパン術を行う方法を考案した。また，外傷性角膜欠損に一次縫合を行い前房を保持し，その後に角膜移植術を行った。このように，角膜穿孔や欠損に前房形成後に全層角膜移植術を行い，視力改善に成功した。

Shallow anterior chamber and soft ocular tension are troublesome when planning penetrating kera-toplasty. We use cyanoacrylate adhesives with scleral strip to seal corneal perforation secondary to sloughing keratitis. The anterior chamber is re-formed, and ocular integrity is restored soon after the sealing. Three cases were successfully treated with penetrating keratoplasty after this procedure.

In a case of corneal laceration with defect, the wound was repaired primarily by direct suturing after trauma.
