(展示126) 36歳女性が流産後に左眼視力低下を主訴として受診した。両眼に網膜色素変性症の所晃があり,左眼眼底の下方に硬性白斑と血管腫に類似する隆起性病変があった。左眼病変部をコーツ病と診断し,色素レーザーによる光凝固を十数回行った。血管腫様の隆起性病変が消退したのち,後極部と眼底上方に広範な血管孤張と滲出性病変を生じたので冷凍凝固を追加した。網膜色素変性症にコーツ病様病変が合併することを示す症例である。
A 36-year-old female presented with blurring in her left eye shortly after spontaneous abortion. Both eyes showed typical features of retinitis pigmentosa. The left eye showed hard exudates and elevated lesions simulating retinal angioma. The left eye was diagnosed as Coats' disease and was treated by repeated photocoagulation with dye laser. After the elevated fundus lesions had subsided, disseminated vasodilatation with exudative lesions appeared in the posterior and superior peripheral fundus in the left eye, necessitating additional cryoapplication. This case illustrates that exudative fundus lesions may develop in association with retinitis pigmentosa.
