We evaluated the incidence of apoptosis-positive cells in the lens epithelium obtained during cataract surgery in 13 eyes. The series comprised 5 eyes with cataract only and 8 eyes with primary angle-closure glaucoma. One eye in the latter group had also exfoliation syndrome. When processed by the TUNEL method, none of the 5 eyes with cataract only showed no positive cells. Among the 8 glaucoma eyes, 3 eyes showed 2 to 17 positive cells among 500 cells. The incidence was the highest in the eye with exfoliation syndrome. The incidence of positive cells was independent of the width of chamber angle, highest value or the duration of elevated intraocular pressure before surgery. The findings show that lens epithelial cells in glaucoma eyes may undergo apoptosis. Elevated intraocular pressure appears to not necessary induce apoptosis due, possibly, to individual susceptibility to intraocular pressure, degree of cataract and difference in elasticity of the lens epithelium.
