We examined 8 eyes of 7 patients who showed spontaneous vitreofoveal separation in the early stages of macular holes with a scanning laser ophthalmoscope. A pseudo-operculum was present immediately anterior to the fovea. Cystoid macular change was evident initially. Posterior vitreous detachment occurred in 4 eyes during the course of observation. During a mean follow up of 7.3 months, the macular lesion improved in 2 and remained stationary in 2 of these 4 eyes. No improvement had set in the other 4 eyes which did not show posterior vitreous detachment. These findings seemed to suggest that it takes more than one year for the macular lesions to improve after posterior vitreous detachment. After vitreofoveal separation, the tangential traction of posterior vitreous membrane to the fovea may be diminished but does not completely disappear.
