　特発性黄斑円孔の症例，26例39眼の網膜硝子体の状態を走査レーザー検眼鏡を用いて観察した。このうち24眼に硝子体手術を行い，術前後の網膜硝子体の状態を比較検討した。アルゴンレーザー光による走査によって，後部硝子体膜の牽引により中心窩の陥凹が消失し，その後網膜の一部に亀裂が入って黄斑円孔が形成されていく様子が観察された。ヘリウムネオンレーザー光による走査では，円孔周囲の剥離と嚢胞性変化がみられた。この2つのレーザー光で観察することにより，黄斑円孔の形成過程に後部硝子体膜の接膜方向の牽引が強く関与していることが証明された。また，硝子体術後は術前の黄斑円孔の時期にかかわらず，中心窩の陥凹が戻り，網膜剥離が消失した。

We observed 39 eyes of idiopathic macular hole using monochromatic scanning laser ophthal-moscope. Vitrectomy was later performed in 24 of these eyes. Observation with argon laser showed disappearance of foveal depression due to traction by posterior vitreous membrane followed by formation of macular hole secondary to partial dehiscence of the retina. Observation with helium-neon laser showed retinal elevation around the hole with cyst formation. The findings showed that tangential traction by the posterior vitreous membrane led to the formation of macular hole. After vitreous surgery, the foveal depression reappeared with flattening of the retina regardless of its stage.
