要約　目的：黄斑円孔が自然閉鎖した後に再発し，後部硝子体剝離がない1例の報告。症例：56歳男性が3週間前からの歪視で紹介受診した。所見と経過：矯正視力は右1.0，左1.2で，光干渉断層計でstage 2の全層黄斑円孔が右眼にあった。後部硝子体剝離はなかった。円孔径が56μmと小さいため，そのまま経過を観察した。円孔は2か月後に自然閉鎖し，視力は1.2に改善した。その8か月後に黄斑円孔が再発し，視力は0.8に低下した。後部硝子体剝離はなく，円孔径は200μmであった。白内障と硝子体手術を同時に行い，円孔は閉鎖し，視力は1.5に回復した。1年後の現在まで経過は良好である。結論：後部硝子体剝離がなく，自然閉鎖した黄斑円孔では，円孔が再発することがあり，注意が必要である。

Abstract.　Purpose：To report recurrence of macular hole after spontaneous closure in an eye with no posterior vitreous detachment. Case：A 56-year-old male was referred to us for metamorphopsia in the right eye since 3 weeks before. Findings and Clinical Course：Corrected visual acuity was 1.0 right and 1.2 left. Optical coherence tomography showed stage 2 macular hole with no posterior vitreous detachment in the right eye. Diameter of macular hole was 56μm. The macular hole closed spontaneously 2 months later with improvement of visual acuity to 1.2. Macular hole reccured another 8 months later with visual acuity reduced to 0.8. Posterior vitreous detachment was absent. Diameter of macular hole was 200μm. Macular hole closed after simultaneous cataract and vitreous surgery. Visual acuity improved to 1.5. There has been no recurrence for one year until present. Conclusion：The present case illustrates that macular hole may recur in an eye with no vitreous detachment and that careful follow-up is needed for eventual recurrence.
