黄斑円孔を伴う黄斑下血腫が生じた網膜細動脈瘤の6例

Macular hole formation in six cases of retinal arterial macroaneurysm with submacular hematoma

要約　目的：黄斑下血腫と黄斑円孔が続発した網膜細動脈瘤6例の報告。症例：過去40か月間に硝子体手術を行った網膜細動脈瘤6例6眼を対象とした。男性1例，女性5例で，年齢は76～82歳である。手術は推定発症日から2週以内に行った。所見：6眼すべてに網膜細動脈瘤と連続する網膜下出血と黄斑円孔があった。中心窩から網膜細動脈瘤までの距離は1.2乳頭径以内であった。5例で0.5以上の最終視力を得た。結論：網膜下出血を伴う網膜細動脈瘤では，黄斑円孔は必ずしも視力予後の不良因子ではない。黄斑円孔は網膜下出血により圧出された可能性がある。

Abstract.　Purpose：To report 6 cases of retinal arterial macroaneurysm with submacular hematoma and macular hole. Cases：This study was made on 6 eyes of 6 cases of retninal arterial macroaneurysm who received vitreous surgery during the past 40 months. The series comprised one male and 5 females. The age ranged from 76 to 82 years. Surgery was performed within 2 weeks after presumed onset. Findings：All the 6 eyes showed macular hole and submacular hematoma continuous with the macroaneurysm. The distance between the fovea and macroaneurysm was less than 1.2 disc diameters. Final visual acuity was 0.5 or better in 5 eyes. Conclusion：These cases illustrate that presence of macular hole is not necessarily a prognostic factor for poor visual outcome. Macular hole may have formed due to expression of submacular hematoma.
