Abstract. Purpose:To report the outcome of surgery for ruptured retinal arterial macroaneurysm as related to the type and location of hematoma. Cases and Method:This study was made on 39 eyes that received vitreous surgery for hemorrhage or macular edema secondary to ruptured macroaneurysm. The series comprised 12 eyes of 12 males and 27 eyes of 26 females. The age ranged from 55 to 92 years,average 75 years. Phakic eyes received phacoemulsification-aspiration with intraocular lens insertion. Cases were followed up for 3 months or longer. Results:Final visual acuity was poorer in 14 eyes with subretinal hemorrhage than in 25 eyes without. In these 14 eyes,final visual acuity was independent on irrigation of subretinal space. Visual acuity improved in eyes with macular edema before surgery. Conclusion:Visual outcome was poorer in eyes with subretinal hematoma. Surgery was also effective for macular edema.
