網膜静脈分枝閉塞症に併発した黄斑浮腫の治療

Treatment of macular edema secondary to branch retinal vein occlusion

網膜静脈分枝閉塞症に併発した黄斑浮腫14眼に対する治療効果を回顧的に評価した。8眼には光凝固を行い，6眼には硝子体手術による後部硝子体膜剝離を作製した。全例で黄斑浮腫が軽快し，視力が2段階以上向上した。光凝固群は硝子体手術群よりも，発症から期間が短く，最終視力が概して良好であった。2眼では光凝固で視力が改善せず，硝子体手術で改善した。発症から7か月以内の網膜静脈分枝閉塞症に併発した黄斑浮腫に対し，光凝固と硝子体手術はともに有効であると結論される。

We reviewed 14 eyes regarding the therapeutic outcome of macular edema secondary to branch retinal vein occlusion（BRVO）. All the eyes received treatment within 7 months after presumed onset of BRVO. Eight eyes were treated by photocoagulation. Six eyes underwent vitreous surgery with artificial posterior vitreous detachment. All the eyes showed improvement of macular edema with increase of visual acuity by 2 lines or more. Two eyes showed improved visual acuity following vitreous surgery after photocoagulation proved futile. Cases treated by photocoagulation had shorter duration after onset and better final visual acuity than those treated by vitreous surgery. The findings show that photocoagulation and vitreous surgery are effective for macular edema secondary to BRVO.
