1994年1月〜95年4月に徳島大学眼科を受診したベーチェット病患者66名のうち,シクロスポリン(cyclosporin A:CYA)投与を受けかつ1年間以上眼発作のない4例について,その投与を中止し経過を観察した。いずれもCYA投与でレーザーフレアー値の平均が15フォトン以下に低下した。CYAの投与中止前1年間のCYAトラフレベル平均が50ng/ml以下の2例は投与中止後もフレアー値平均は15フォトン以下であったのに対し,50ng/ml以上の2例は15フォトン以上を示した。CYA投与で1年以上自覚的,他覚的な眼発作が抑制され,かつその1年間のCYAトラフレベル平均が50ng/ml以下であれば,CYA療法からの離脱が可能と思われた。
We treated 66 patients of Behçet disease during a 16-month period. There were 4 cases who were treated by systemic cyclosporin A and who were free of ocular attacks for one year or more. We evaluated the clinical course of these 4 cases after withdrawal of cyclosporin A. The mean level of flare was maintained at less than 15 photon counts/msec during treatment. After withdrawal, there was no active ocular inflammation in 2 patients who had a mean level of blood cyclosporin A at less than 50 ng/ml. The findings imply that systemic cyclosporin A can be terminated without recurrence of ocular inflammation when the patients have been free of ocular attacks for at least one year and when their mean blood level of cyclosporin A has been maintained at less than 50 ng/ml.
