Heidelberg retina tomographを用いた緑内障眼の視神経乳頭解析—乳頭パラメータと視野体積との相関

Correlation between optic disc parameters and visual field in glaucoma by laser scanning tomography

　Heidelberg Retina Tomograph （HRT）を用いて緑内障患者30例51眼の視神経乳頭を解析した。得られた網膜高のプロフィールから，51眼を通常の緑内障性陥凹を示す乳頭群（非傾斜乳頭群）25眼と近視性傾斜乳頭群26眼に分け，それぞれの群の乳頭パラメータとゴールドマン視野（GP）の体積との相関係数を求めた。非傾斜乳頭群では，相関係数はC／Dratioで0.719,cup areaで0.718,cup volumeで0.709と比較的強い相関がみられたが，傾斜乳頭群では，すべてのパラメータで相関は弱かった。緑内障の進行度を知る上で，HRTで得られた乳頭パラメータを用いる場合は傾斜乳頭の有無を考慮する必要がある。

Optic discs in 51 glaucomatous eyes were analyzed using a confocal scanning laser ophthalmo-scope, Heidelberg Retina Tomograph. The patients were divided into two groups: those with myopic tilted disc and those with non-tilted disc. We evaluated the relationship between the optic disc parameters and the volume of visual field using Goldmann perimeter. The C/D ratio, cup area and cup volume were highly correlated with the visual field volume in non-tilted discs and not in tilted discs. The findings show the importance to determine the form of the optic disc before evaluating the stage of glaucoma using optic disc parameters with the confocal scanning laser ophthalmoscope.
