アクリル製眼内レンズの挿入後に生じたcapsular block症候群

Capsular block syndrome after insertion of acrylic intraocular lens

　白内障超音波乳化吸引術およびアクリル製眼内レンズ移植後に，capsular block症候群が2例に生じた。2例ともに，術前の前房の深さは正常であった。術翌日に浅前房となり，眼内レンズが嚢内の最前方に移動し，後嚢と眼内レンズとの間に間隙を生じ，後嚢が後方に拡張していた。術創からの房水の漏出はなく，眼圧も正常であった。YAGレーザーによる後嚢切開を行った。直後から前房が深くなりはじめ，3日後に近視度は術前の予想値近くまでに改善した。眼内レンズによる水晶体嚢ブロックは，アクリル製眼内レンズでも起こりえた。

Two eyes developed capsular block syndrome following phacoemulsification and implantation of intraocular lens (IOL) made of soft acryl material. The anterior chamber depth were normal before surgery. The next day after surgery, the anterior chamber became shallow and the IOL had moved forward to the farthest point possible in their capsular bag. A space had formed between the posterior capsule and the IOL, straining backwards. There was neither leakage through the incisional wound nor abnormal intraocular pressure. Posterior capsulotomy with YAG laser induced immediate deepening of the anterior chamber, followed by reduction of myopia to the expected value three days later.
