遺伝子解析で診断された眼窩悪性リンパ腫の1例

Diagnosis of a case of orbital malignant lymphoma by genetic molecular analysis

　遺伝子解析で診断できた眼窩原発悪性リンパ腫の49歳男性を経験した。左上眼瞼腫脹を主訴に来院した。臨床症状，画像検査，病理学的検査で悪性リンパ腫が疑われたが確定診断に至らず，サザンブロッティング法を用いた遺伝子学的検索を施行した。正常健常人対照DNAでは認められない再構成バンドが確認でき，B細胞性悪性リンパ腫と診断された。リンパ系の腫瘍性増殖である悪性リンパ腫は，反応性増殖による病変との鑑別に苦慮することがあり，遺伝子解析を用いた診断法の有用性を報告した。

A 49-year-old male presented with swelling of left upper eyelid. Malignant lymphoma of the orbit was suspected but not confirmed by clinical features, image analysis and pathological examinations. Analysis of biopsied specimen for immunoglobulin genes by southern blot hybridization showed rearrange-ment bands as distinct from germ-line bands of normal DNA and led to the diagnosis of malignant lymphoma of type B cell. Genetic molecular analysis was thus useful in differentiating malignant lymphoma from reactive lymphocytic proliferation.
