先天無虹彩の隅角の組織所見

Morphological features of trabecular tissue in congenital aniridia

　先天無虹彩5例7眼の隅角および周囲組織の病理所見を光学顕微鏡で観察した。いずれも全身合併症があり，早期死亡例のため緑内障の有無は不明であった。全例で隅角線維柱帯の形成不全があり，2眼で線維柱帯細胞は配列が不規則であった。5眼は形成不全が高度で，充実組織となっていた。シュレム管は2眼では存在していなかった。管腔がみられた5眼のうち，1眼はほぼ正常であり，1眼は狭小であった。3眼は細い管腔が多数存在し，通常よりは後方やや外側に広く分布していた。したがって重篤な先天無虹彩では，シュレム管にまでおよぶ隅角組織の発生異常が存在し，これが早期に発症する緑内障の予後不良の原因となるものと推測された。

We performed postmortem histopathological studies of the trabecular tissue in 7 eyes of 5 infants of congenital aniridia. All the eyes showed developmental disorders in the trabecular meshwork. Irregular arrangement of trabecular sheets was present in 2 eyes. Mesenchymal cells filled the poorly developed trabecular meshwork in 5 eyes. Schlemm's canal was present in 5 eyes and absent in 2. It was narrow in 1 eye and was irregularly branched in 3. The findings seemed to suggest that anomalies in the trabecular meshwork and Schlemm's canal would result in glaucoma in eyes with congenital aniridia at an early age.
