We evaluated the visual outcome in 38 eyes in 27 patients who underwent cataract surgery with multifocal intraocular lens .insertion during a 25-month period. They were divided into two groups: those less than 65 years and those aged 65 years or over. Cases were excluded who manifested eye lesions other than cataract. Corrected far visual acuity of 1.0 was achieved in 93% in the younger age group and in 40% in the elder group. Near visual acuity of 0.8 with correction for distance was achieved in 89% in the younger age group and in 20% in the elder group. These differences were significant. We also calculated the logarithmic difference between distant and near visual acuity in each eye. The value averaged 0.13±0.12 for the younger group and 0.67±0.65 for the elder one. The difference was significant. The findings show that near vision tended to be poorer in elder patients than in younger ones. Good near vision was also more difficult to attain than distant vision in elder patients.
