アトピー性皮膚炎に伴う網膜剥離における硝子体中の色素塊

Intravitreal pigment clusters in eyes with retinal detachment associated with atopic dermatitis

　京都大学医学部眼科学教室で手術を施行したアトピー性皮膚炎に伴う網膜剥離35眼について検討した。硝子体基底部の色素塊がアトピー性皮膚炎に伴う網膜剥離の40％でみられ，これは外傷により生ずる色素塊と酷似していた。アトピー性皮膚炎の患者では，掻痒のため眼部をたたく，もしくはこするという長期にわたる慢性外傷がきっかけとなって特徴的な色素塊が形成されることがあると考えられた。この色素塊もしくは外傷による慢性炎症による硝子体の索引が，アトピー性素因とあいまって網膜裂孔を形成もしくは悪化する場合があると考えられた。

We reviewed a series of 35 eyes with retinal detachment associated with atopic dermatitis. Characteristic pigment clusters were present in the vitreous base in 14 eyes, 40 %. The pigment clusters simulated those that appeared after blunt eye injury. Patients with atopic dermatitis tend to slap over the eyes due to itchy sensations. This repeated eye injury is a possible cause of the observed pigment culsters. Further, the vitreous traction caused by these clusters or by chronic ocular inflammation by eye slappings may be one of the causes of retinal tears in patients with atopic dermatitis.
