アトピー性皮膚炎に伴う網膜剥離

Retinal detachment associated with atopic dermatitis

　アトピー性皮膚炎に網膜剥離を合併した3症例の臨床所見について報告した。

　3例とも10代の男性で，症例1のみアトピー性白内障術後であった。網膜剥離は症例2のみ両眼性で，3例とも鋸状縁断裂を認め，症例2，症例3では色様体上皮剥離を伴っていた。症例3では両眼とも毛様体扁平部に嚢胞を認め，網膜剥離を認めなかった左眼では，経過中に，嚢胞部に一致して鋸状縁断裂を生じた。

　アトピー性皮膚炎に網膜剥離を合併した症例は，現在までに22例31眼の報告があり，これに自験例を加えた25例36眼の臨床的特徴について検討するとともに，その原因について若干の考察を加えた。

Three cases with retinal detachment associated with atopic dermatitis were reported. One case was unilaterally and the other cases were bilaterally involved. Atopic cataract was present in all of these cases. Retinal detachment occurred after cataract surgery in one case, and spontaneously in two cases. In all three cases oral dialysis was present. Detach-ment of ciliary epithelium in two cases and pars plana cysts in one case were present.

We reviewed the literature and found that oral dialysis was one of the commonest features of reti-nal detachments associated with atopic dermatitis.
