ガス注入術による近視性黄斑円孔網膜剥離の復位率に影響する諸因子

Factors for reattachment rate in retinal detachments with myopic macular hole treated by gas tamponade

　近視性黄斑円孔網膜剥離に対して，初回手術としてガス注入術あるいはガス注入術＋レーザー光凝固術を施行した58例58眼について治療成績に影響する諸要因を検討した。（1）初回復位率は45％，最終復位率は90％であった。（2）初回復位率が不良な要因はガス滞留時間が8日以下，屈折度が-13.0Dより強い近視，網膜剥離の範囲が後極部に限局するもの，後部ぶどう腫，無水晶体眼であった。（3）最終復位率が不良な要因は屈折度が-20.0Dより強い近視と後部ぶどう腫であった。

We reviewed the surgical outcome in 58 myopic eyes with retinal detachment due to macularhole treated by gas tamponade as the initial procedure. The retina reattached in 45% of eyes after initialsurgery and in 90% after additional procedures.Following factors were significantly involved for failureafter the initial procedure:duration of gas retention less than 8 days, localized retinal detachment withinthe posterior fundus, aphakia, myopia greater than -13 diopters, and presence of posterior staphyloma.Thelatter two factors also affected the final failure of detachment surgeries.
