外側後脈絡叢動脈閉塞による外側膝状体梗塞の1例

An unusual case of infarction of lateral geniculate body due to lateral choroidal artery occlusion

　外側後脈絡叢動脈閉塞による外側膝状体梗塞の1例を経験した。57歳，男性。突然の両眼霧視を自覚し，当科受診した。翌日当科入院となり，ゴールドマン視野計とオクトパス自動視野計による視野検査で黄斑部回避を伴わない特有の扇形の左同名性半盲を認めた。CTでは視覚路には特に異常がなかったが，MRIではT2強調画像にて右外側膝状体領域に限局した巣状の高信号強度像が描出された。MRIの所見と視野欠損の形状から外側後脈絡叢動脈の閉塞による脳梗塞と診断した。

We observed a case of lateral geniculate body infarction induced by lateral choroidal artery occlusion. A 57-year-old man presented with blurred vision in both eyes. After admission, we evaluated the Goldmann perimeter and Octopus perimeter, and found left congruent, wedge-shaped homonymous hemianopsia. A brain CT scan did not disclose an ischemic lesion, but a brain MRI scan clearly disclosed a high intensity mass in the right lateral geniculate body area. From these observations, we made a diagnosis of brain infarction induced by lateral choroidal artery occlusion.
