外側膝状体出血により水平区画半盲を呈した1例

Homonymous horizontal sectoranopia in a case after hemorrhage in the lateral geniculate body

65歳女性が脳内出血を発症した。同時に両眼霧視を自覚し，その10日後に受診した。視力は正常で，前眼部と眼底に異常所見はなかった。視野検査で固視点から水平経線を挟んで左方に広がる同名性視野障害があった。頭部の画像診断で右側の外側膝状体部に出血巣が発見され，これによる水平区画半盲と考えられた。この視野障害の成因には，視索から外側膝状体を経て前部視放線に至る神経線維の走行と，外側膝状体での血管支配の形態が関与している。

A 65-year-old woman had an attack of cerebral hemorrhage followed by blurring of vision in both eyes. When seen 10 days later,she had full visual acuity. Routine ophthalmological examinations showed no abnormal findings. Perimetry showed,in both eyes,wedge-shaped homonymous hemianopia,or homonymous horizontal sectoranopia. Computerized tomography（CT）and magnetic resonance imaging（MRI）showed hemorrhage involving the right lateral geniculate body as the presumed cause of visual field disorder.
