小児軽度近視例における4％ホマトロピン加1％サイクロジル液点眼の調節麻痺作用

Cycloplegia with 4% homatropine and 1% cyclogyl in children with low-grade myopia

　調節の異常緊張を除去して屈折検査を行い，眼鏡を処方する目的で，4％ホマトロピン加1％サイクロジル液を小児近視例に点眼した。1％サイクロジル液単独よりも有意にプラス側の自覚的屈折値を得たが，その差は平均値で0.001Dと小さく，雲霧下と同等の自覚的屈折値を得た。調節麻痺作用が最長5.9日も持続し，点眼痛が強かったことを総合すると，4％ホマトロピン加1％サイクロジル液を今後小児近視例の第一次選択薬剤として使用することは妥当ではなく，小児近視例の屈折検査においては，雲霧法が第一次選択として用いられるべきものと考えた。

We compared the cycloplegic effect of topical instillation of 4% homatropine and 1% cyclogyl with 1% cyclogyl alone in 21 children with low-grade myopia. The ages averaged 10.7 ± 2.4 years. The degree of myopia ranged from -0.25 D to -3.25 D, average -1.8 ± 0.9 D. The power of refraction under cycloplegia was identical between the two methods. Combined instillation induced prolonged cycloplegia for 5.9 days and considerable ocular pain. The findings show that combined ophthalmic solution of 4% homatropine and 1% cyclogyl is not adequate for examination of refraction in children with low-grade myopia. Fogging method is advocated to exclude abnormal tonic accommodation.
