We compared the cycloplegic effect of topical instillation of 4% homatropine and 1% cyclogyl with 1% cyclogyl alone in 21 children with low-grade myopia. The ages averaged 10.7 ± 2.4 years. The degree of myopia ranged from -0.25 D to -3.25 D, average -1.8 ± 0.9 D. The power of refraction under cycloplegia was identical between the two methods. Combined instillation induced prolonged cycloplegia for 5.9 days and considerable ocular pain. The findings show that combined ophthalmic solution of 4% homatropine and 1% cyclogyl is not adequate for examination of refraction in children with low-grade myopia. Fogging method is advocated to exclude abnormal tonic accommodation.
