若年者にみられ多発性脳梗塞を伴った低眼圧緑内障の1例

Low-tension glaucoma and asymptomatic multiple cerebral infarction in a young adult

　無症候性脳梗塞を有する女性若年者例に発症したきわめてまれな低眼圧緑内障の1例を，循環障害説をあげながら報告した。症例は23歳女性。両眼の視力低下，視野欠損を主訴とし，緑内障性乳頭陥凹と視野異常の既往とともに来院。眼圧が終日16mmHg以下，開放隅角，視神経乳頭蒼白，水平上半盲（右＞左）を呈しており，低眼圧緑内障と診断した。T₂強調画像から多発性脳梗塞が見いだされたが，頭蓋内腫瘍は否定され，虚血性視神経症は螢光眼底所見などから否定された。オリゴクロナールバンド正常，Uhthoff test陰性，安定した症状などから多発性硬化症も否定された。発症要因からみると本例は若年例，女性，高血圧ならびに糖尿病がないことなどからきわめてまれな低眼圧緑内障の症例であると思われた。

A 23-year-old female presented with bilateral visual disturbance and field defects. Intraocular pressure was persistently below 16 mmHg. Both eyes showed open angle, pale optic disc and horizontal hemianopia suggestive of low-tension glaucoma. Magnetic resonance imaging (MRI) showed multiple cerebral infarctions through T₂ weighted images. Further studies ruled out intracranial tumor or ischemic optic neuropathy. This patient was thus a very unusual instance of low-tension glaucoma secondary to asymptomatic multiple cerebral infarction in spite of her age and sex and of absent systemic hypertension or diabetes mellitus.
