視神経疾患におけるビタミンB12動態

Involvement of vitamin B12 in optic nerve disorders

　視神経疾患のビタミンB₁₂（B₁₂）動態を知る目的で特発性視神経炎8例，虚血性視神経症3例を中心に，radioimmunoassayを用いて血清および髄液中のB₁₂およびB₁₂不飽和結合能の定量を行った。各症例とも血清B₁₂濃度には異常は認められなかったが，髄液中B₁₂濃度は特発性視神経炎では75％の症例で10pg／ml未満の低値を示し，対照的に虚血性視神経症では異常は認められなかった。これらの相違は視神経障害の発症様式や臨床経過の差を反映していると考えられたが，特発性視神経炎では血清B₁₂濃度では予想できないような神経組織のB₁₂減少が起こっている可能性が示唆された。

We quantitated the level of vitamin B₁₂ and its unsaturated binding capacity in the serum and cerebrospinal fluid in 14 cases of optic nerve disorders. The series comprised idiopathic optic neuritis 8 cases, ischemic optic neuropathy 3, toxicity from inhalation of organic solvent 2 and Leber's hereditary optic atrophy 1. Significant reduction of B₁₂ in the cerebrospinal fluid under 10pg/ml was present in 6 cases of optic neuritis, 75%. No such finding was observed in cases of ischemic optic neuropathy. These differences seemed to reflect the various causes and clinical features of optic nerve disorders.
