(17-G409-16) 頸動脈海綿静脈洞瘻(CCF)の結膜充血に対して,レーザースペックル法を用いて結膜血流を測定し経過を観察した。結膜血流はsquare dur rate(SBR)値にて評価した。症例は76歳女性,CCFに対し塞栓術が行われ,症状は改善し結膜血流測定にてSBR値は左右差はなかった。3か月後に再発し右眼SBR値は再発前の7倍まで上昇した。再度塞栓術が行われたが症状およびSBR値に変化なく,放射線治療を行い右眼SBR値は再発前と同程度まで低下した。この症例では,結膜血管のSBR値は結膜充血,眼圧の経過と相関性を示した。同装置を用いることにより結膜血流を定量的に測定し,充血の程度を評価することが可能になった。
A 76-year-old woman developed carotid cavernous fistula (CCF). Embolization of CCF induced disappearance of conjunctival congestion in her right eye. We quantitated the conjunctival blood flow using a laser speckle flowmeter using the square blur rate (SBR) as parameter. There was no difference in SBR value between both eyes. After relapse of CCF 3 months later, the SBR increased to 700% of the former value. Retreatment of CCF by embolization failed to influence the clinical features and SBR value. Radiation to CCF decreased the SBR value to the initial level. Throughout the course of observation, the SBR values were positively correlated to the severity of conjunctival congestion and intraocular pressure. This case illustrates the usefulness of SBR value by laser speckle flowmetry as parameter of blood flow rate in the conjunctiva.
