特発性黄斑円孔の走査レーザー検眼鏡による螢光造影所見

Fluorescein angiographic features of idiopathic macular holes using scanning laser ophthalmoscope

　特発性黄斑円孔の硝子体手術の前後に走査レーザー検眼鏡（SLO）を用いたフルオレセイン螢光造影（FA）をおこない，手術前後の造影所見について検討した。術前のFAでは，4例中3眼に嚢胞様黄斑浮腫（CME）様の花弁状過螢光を認めた。術後のFAでは，CME様の花弁状過螢光はすべて消失した。この術前のCME様の花弁状過螢光は円孔周囲のcystic spaceの造影所見と考えられ，術後花弁状過螢光が消失したことから，この造影所見は硝子体による網膜牽引の証拠であると思われた。SLOを用いたFAは特発性黄斑円孔の硝子体手術の成果ならびにその病態を検討する上で有用であると結論された。

We treated four eyes of idiopathic macular hole by vitreous surgery. Fluorescein angiography was performed before and after surgery using a scanning laser ophthalmoscope. Cystoid macular edema was present around the hole showing petalloid hyperfluorescence in 3 eyes. The petalloid hyperfluorescence disappeared after surgery. The finding seemed to suggest that the petalloid hyperfluorescence shows the presence of cystic spaces in the close vicinity of macular holes and the evidence of vitreoretinal traction to the fovea.
