We evaluated the effect of topical apraclonidine, an α-2 adrenergic agonist, on the intraocular pressure (IOP) after retrobulbar anesthesia with 4 ml of 2% mepivacaine. A series of 18 cataract eyes served as test group with another 17 eyes serving as control. A Honan IOP reducer was applied for 5 minutes at 40 mmHg following retrobulbar anesthesia. The IOPs before instillation, before and after retrobulbar anesthe-sia and after removal of Honan reducer averaged 16.2, 15.8, 18.0 and 13.4 mmHg respectively in the test group and 16.1, 18.5, 24.9 and 16.6 mmHg in the control group. The latter three values were significantly less in the test than in the control group (p<0.01). The findings indicate that preoperative instillation of apraclonidine decreases the IOP before cataract surgery.
