　臨床所見より低眼圧緑内障（LTG）を疑った60例118眼に対して，診断確定のため24時間眼圧日内変動測定を施行し，その結果を，外来受診時の眼圧値と比較することにより，LTGにおける日内変動測定の臨床的有用性を検討した。日内変動の結果からLTGを否定された症例は4例7眼（6.7％）であった。これらの外来眼圧の平均は17.8±1.5mmHgで，LTGと診断された症例の外来眼圧15.5±2.0mmHgと比較して有意に高かった（p＜0.01）。LTGと診断された各症例の外来眼圧と日内眼圧の差（カッコ内は両者の相関係数）は，最高眼圧では0.8±1.9mmHg （0.58），最低眼圧では1.1±1.7mmHg （0.65），平均眼圧では1.2±1.3mmHg （0.79）と，いずれも外来眼圧が有意に高かった（p＜0.001）。また，外来平均眼圧と，外来診療時間に相当する午前10時から午後4時の間における眼圧日内変動平均との差（相関係数）も，1.2±1.4mmHg （0.60）と，前者が有意に高かった（p＜0.001）。この結果より，LTGの日内変動測定値は外来受診時の眼圧値と比較して約1mmHg低いレベルにあると考えられ，かつその24時間の平均値はかなりの精度で外来眼圧の平均データより推定することができると考えられた。

We evaluated the clinical value of 24-hour-around-the-clock intraocular pressure (IOP) mea-surement in 118 eyes of 60 patients suspected of low tension glaucoma (LTG). The findings were compared with IOP readings in the outpatient clinic (out-IOP) in the same group. Seven eyes in 4 cases manifested IOP readings of 22mmHg or higher by 24-hour IOP measurement and were excluded from the LTG group. These 4 patients showed mean I0P of 17.8±1.5mmHg, which was significantly higher than that in the remaining 56 LTG patients, 15.5±1.3mmHg (p＜0.01). In the LTG group, the out-IOP was significantly higher than the 24-hour I0P regarding the peak, the trough and the mean by 0.8±1.9, 1.1±1.7 and 1.2±1.3mmHg, respectively (p＜0.001). The correlation coefficients between the out-I0P and 24-hour IOP (r) were 0.58 for the peak, 0.65 for the trough, and 0.79 for the mean, respectively. Furthermore, the mean IOP in the outpatient clinic was also significantly higher than the mean 24-hour IOP by 1.2±1.4mmHg (p＜0.001, r=0.60). The ordinary IOP in LTG cases fluctuated at a level higher by 1mmHg than the estimated value by 24-hour IOP measurement. This finding seemed to be of clinical significance in deciding the modality of treatment for LTG.
