Effect of bumetanide on the intraocular pressure

緒　言

　緑内障に対する全身投与の眼圧下降剤は，炭酸脱水酵素阻害剤，高滲透圧剤等があるが，これらの薬剤については，なお，満足し得る結果を得ないこともあり，更に新しい眼圧下降剤の開発が望まれている。

　今回，われわれは三共株式会社より新しい利尿剤で，主としてヘンレ係蹄を含む遠位部ネフロンに作用し，この部位のNaの再吸収を抑制することにより強力な利尿作用を有する^1）というBumetanide（商品名Lunetoron）を眼圧下降の目的で使用する機会を得て，これが著しい効果を示すことを経験したのでここに報告する。

Sixteen normal and 17 glaucomatous subjects (9 primary and 8 secondary glaucomas) were given 1 mg of bumetanide (3-n-butylamino-4- phenoxy-5-fulfamoyl benzoic acid) perorally. Bumetanide is a potent diuretic and we measured the intraocular pressure before and after the peroral administration up to 24 hours.

A significant decrease in intraocular pressure in normal subjects occurred 90 minutes after administration and lasted at least 90 minutes thereafter. In glaucomatous subjects, the hypo-tensive effect became manifest 30 minutes after administration.
