糖尿病網膜症7年間の追跡調査第2報HbA1値との関連

A seven-year follow up of diabetic retinopathy. Report 2. Implication of serum hemoglobin A1 level

　1984年〜1990年の7年間，都立豊島病院内科および眼科で経過観察したインスリン非依存型糖尿病患者75名150眼の網膜症とHbA₁値を追跡調査した。1）網膜症なし群は単純網膜症群に比し有意にHbA₁値が低かった。網膜症発症率は，HbA₁値が高値になるに従い高くなる傾向を示した。発症群は非発症群に比し，3年後，5年後は有意にHbA₁値が高値であったが，7年後は有意差がなかった。2）単純網膜症群の悪化率はHbA₁値が高値になると有意に高くなった。悪化して増殖網膜症になった群は，他の単純網膜症群に比し有意にHbA₁値が高値だった。3）増殖網膜症群では予後とHbA₁値との間に明らかな関連はなかった。

We followed up the clinical course of retinopathy in 75 insulin-nondependent diabetic patients over a 7-year period through 1990. Initially, the series comprised 68 eyes without retinopathy, 57 with simple retinopathy and 25 with proliferative retinopathy. Patients without retinopathy showed significantly lower levels of serum HbA₁ than in those with simple retinopathy. The HbA₁ value was positively correlated with the incidence of retinopathy. At 3 and 5 years after start of follow-up, patients who developed retinopathy showed higher HbA₁ value than those without retinopathy. At 7 years, there was no difference in the value between the two groups. High HbA₁ value was positively correlated with deterioration of simple retinopathy and with developing proliferative retinopathy from simple one. HbA₁ value was not correlated with the fate of proliferative retinopathy.
