We followed up the clinical course of retinopathy in 75 insulin-nondependent diabetic patients over a 7-year period through 1990. Initially, the series comprised 68 eyes without retinopathy, 57 with simple retinopathy and 25 with proliferative retinopathy. Patients without retinopathy showed significantly lower levels of serum HbA1 than in those with simple retinopathy. The HbA1 value was positively correlated with the incidence of retinopathy. At 3 and 5 years after start of follow-up, patients who developed retinopathy showed higher HbA1 value than those without retinopathy. At 7 years, there was no difference in the value between the two groups. High HbA1 value was positively correlated with deterioration of simple retinopathy and with developing proliferative retinopathy from simple one. HbA1 value was not correlated with the fate of proliferative retinopathy.
