糖尿病網膜症7年間の追跡調査第1報積極的治療を行った一般病院での予後

A seven-year followup of diabetic retinopathy. Part 1. Clinical course after intensive therapy in urban hospitals

　1984〜1990年の7年間に，前増殖性網膜症早期より積極的に光凝固を行うという治療方針で管理したType 2糖尿病患者75例150眼の網膜症自験例を追跡調査した。7年後に網膜症のない群は減少，単純性網膜症群は増加し，増殖性網膜症は増減がなかった。網膜症のない群のうちそのまま残ったのは44.1％であった。福田分類A2のうち，A2のままだったのは52.3％であり，増殖化したのは34.1％であった。全体では，改善16.0％，不変40.0％，悪化44.0％であり，従来の大学病院の報告と大差ない結果であった。大学病院では理想的管理が行われ網膜症の予後がよいと考えられるが，一般病院でも積極的に光凝固を行えば大差ない結果が得られることがわかった。

We reviewed the clinical course of diabetic retinopathy in 150 eyes, 75 patients with type 2 diabetes. All the eyes were actively treated by photocoagulation. At the end of 7 years, the incidence of negative retinopathy decreased, that of simple retinopathy increased and that of proliferative retinopathy remained unchanged. Out of 44 eyes with initially stage A2 retinopathy according to Fukuda, 52% remained in the same category and 34% developed proliferative retinopathy. As an overall evaluation, the retinopathy improved in 16%, remained unchanged in 40% and deteriorated in 44%. Above results, which were obtained by general hospitals in Metropolitan Tokyo, are similar to those reported from university eye clinics.
