緑内障眼に対する超音波乳化吸引術を用いた白内障同時手術

Simultaneous trabeculotomy and phacoemulsification for glaucoma with cataract

　緑内障を伴った白内障に対する超音波乳化吸引術，後房レンズ挿入術，トラベクロトミー，またはそれらにシヌソトミーをさらに併用した同時手術を行った38眼について報告した。術後眼圧コントロールは術後3か月以降では全例で良好であった。術後視力は0.5以上が86％であり，術後視力が低下した症例はなかった。術後2週間で視力1.0以上が43％，術後1か月では67％に認められ，術後早期から良好な視力が得られた。術後併発症は，30mmHg以上の一過性眼圧上昇が36％あり，これらはすべて1週間以内に出現し，薬物療法で改善した。術中・術後併発症では脈絡膜下出血，デスメ膜血腫を1例ずつ認めたが，この同時手術は有効な術式と考えられた。

We performed phacoemulsification, posterior chamber lens implantation and trabeculotomy in 38 eyes with glaucoma and cataract. The series included open angle glaucoma 28 eyes, combined glaucoma 6, secondary glaucoma 2 and low tension glaucoma 2. The intraocular pressure was well controlled after 3 months in all eyes with or without medication. The postoperative visual acuity was 0.5 or better in 86% of eyes. No eye showed decrease of vision after surgery. Transient ocular hypertension over 30 mmHg devoloped in 13 eyes, 36%. The current simultaneous surgical procedure was safe and effective for glaucomatous eyes with cataract.
