　有水晶体眼の隅角癒着解離術（GSL）におけるレーザー隅角形成術（LGP）併用の効果と適応とについて検討した。併用例での有効率は92％であった。少数例で術後再癒着が発生したが，その程度は強くなく範囲も限局していた。LGPによる併発症はなかった。これは，GSLに水晶体摘出術を併用した成績とほぼ同等であり，良好な結果であった。従ってGSLと水晶体摘出術の併用は隅角検査で周辺虹彩前癒着の範囲が広く高い症例や，水晶体赤道部を透して毛様突起がみられるなど，水晶体の強い前方移動や膨隆を示す所見がある症例に，少なくとも最初は限られるべきである。

We performed goniosynechialysis with laser gonioplasty in 21 eyes with closed angle glaucoma. The series comprised 13 phakic eyes (group A) and 1 aphakic and 8 pseudophakic eyes (group B). The follow-up period averaged 19.9±8.2 months for group A and 12.2±8.9 months for group B. After treatment, the intraocular pressure was controlled below 21mmHg in 12 eyes in group A and in 8 in group B with or without topical medications. There was no difference in the success rate between the two groups. The postoperative incidence of peripheral anterior synechia was 38% in group A and 16% in group B. The findings indicate that the present method is effective for control of intraocular pressure even in phakic eyes.
