We treated 5 glaucoma eyes by sclerostomy ab externo using THC-YAG laser which emits pulsed laser at 2100nm wavelength. The intraocular pressure was controlled below 20mmHg with or without topical therapy in 4 eyes, 80%, at 12 months after treatment. In 2 eyes with narrow angle, the iris prolapsed into the sclerostomy site resulting in absent filtering bleb. In one eye with neovascular glaucoma, the filtering bleb disappeared soon after treatment. As a major complication, astigmatism with folds in Descemet's membrane developed in all eyes secondary to burn at the treated sclerocorneal site. The findings seemed to show that sclerostomy ab externo with THC-YAG laser is indicated for patients with open angle glaucoma under poor systemic conditions.
